Résumé
Les auteurs rapportent le cas d’une patiente de 12 ans admise aux urgences pour une détresse respiratoire de survenue brutale. La radiographie thoracique faisait évoquer un hydropneumothorax gauche compressif. Immédiatement, un drainage thoracique a été réalisé. Cependant devant l’apparition d’une douleur thoracique permanente et d’un liquide de drainage bilieux, le scanner thoracique demandé a confirmé la présence de l’estomac avec le drain in situ dans la cavité thoracique gauche. Une thoracotomie gauche en urgence a confirmé le diagnostic et a permis de réaliser une gastrorrhaphiesuivie de la réduction de la hernie et la fermeture de l’orifice diaphragmatique. Nous avons noté une bonne réexpansion pulmonaire post-opératoire. L’évolution a été simple.

Abstract
The authors report the case of a 12-year-old female patient admitted to the emergency department for sudden onset respiratory distress. Chest X-ray suggested a left compressive hydropneumothorax. A thoracic drainage was performed immediatly. However, faced with the occurence of permanent chest pain and bilious drainage fluid, the thoracic CT scan requested revealed the presence of the stomach with the drain in situ in the left thoracic cavity. An emergency left thoracotomy confirmed the diagnosis and allowed gastrorrhaphy followed by hernia reduction and closure of the diaphragmatic orifice. We noted a good postoperative pulmonary re-expansion. The evolution was straightforward.
