Résumé :
Les auteurs rapportent le cas d’un patient de 58 ans qui a présenté un syndrome coronarien aigu avec à l’électrocardiogramme un sus décalage du segment ST en territoire circonférentiel. Une revascularisation par streptokinase a été faite avec succès. La coronarographie réalisée ensuite a montré une lésion significative de l’artère interventriculaire antérieure (IVA). Les investigations ont mis en évidence un terrain de polyglobulie vraie. Compte tenu de l’hypercoagulabilité observée chez ce type de patient, la décision de ne pas stenter la lésion coupable a été prise. Le patient a donc été mis sous thérapeutique cytoréductrice à base de phlébotomie et d’hydroxyurée. Après 2 ans de suivi aucun évènement thrombo-embolique n’a été signalé.

Abstract :
The authors report the case of a patient of 58 years, who presented acute coronary syndrome with an elevated S-T segment in circonferential territory on electrocardiogram. Revascularization by streptokinase has been made successfully. Coronary angiography performed then showed a significant lesion of the anterior descending artery (ADA). Investigations pointed out a polycythemia vera field. Considering the hypercoagulability observed in such patients, the decision was taken not to stent the ADA lesion.The patient received cytoreductive therapeutic with phlebotomy and hydroxyurea. After two years of follow-up the patient was still free of thromboembolic events.
