Résumé :

Les auteurs rapportent le cas d’une patiente de 64 ans, non fumeuse, admise au service de pneumophtisiologie du CHU du point G dans un tableau de toux chronique productive, douleurs thoraciques prédominantes à gauche, dyspnée d’effort évoluant depuis 56 mois. La radiographie thoracique a montré un syndrome alvéolaire diffus. Le scanner a révélé de multiples nodules, diffus et bilatéraux évoquant une granulomatose. Une biopsie à thorax ouvert a permis un examen anatomopathologie du prélèvement qui a montré des cellules à noyau atypique et hautement mucosécrétantes avec infiltration des cloisons interalvéolaires, compatible avec un carcinome bronchiolo-alvéolaire invasif. Il s’agit du premier cas rapporté dans le service.

Abstract :

The authors report the case of a 64 years non smoker woman admitted in pneumophtisiology department at CHU du point G. She had a chronic productive cough, a left predominant chest pain, an exertional dyspnea for 56 months. Chest x ray showed a diffuse alveolar syndrome. CT scanner revealed multiple, diffuse and bilateral nodules evoking granulomatosis. Open chest biopsy allowed a levy pathological examination which showed atypical nucleated and highly mucus secreting cells with infiltration interalveolar partitions, consistent with invasive bronchioloalveolar carcinoma. This is the first reported case in the department.
