Résumé :
Le but de ce travail était de rapporter les résultats de la chirurgie des goitres endothoraciques dans notre
service. De Février 2001 à Avril 2008 quatre femmes âgées respectivement de 36, 37, 43 et 48 ans ont
été opérées d’un goitre endothoracique euthyroidien. Trois patientes présentaient une tuméfaction
cervicale antérieure et une avait eu une chirurgie thyroïdienne 5 ans auparavant. Chaque patiente
présentait au moins un signe médiastinal de compression mécanique à type de toux sèche, de dyspnée
modérée, de douleur thoracique intermittente, de dysphagie aux aliments solides et de dysphonie. Toutes
ces patientes ont eu une radiographie standard du thorax et un scanner cervico-thoracique confirmant le
diagnostic. Le traitement chirurgical a consisté en une tumorectomie par cervico-sternotomie une fois,
sternotomie médiane verticale une fois, thoracotomie postéro latérale deux fois. Les suites opératoires
immédiates étaient simples chez 3 patientes et marquées par une dysphonie persistante dans un cas. La
mortalité a été nulle. L’examen anatomopathologique des pièces opératoires était en faveur d’un
adénome bénin de la thyroïde. Cette courte série homogène portant sur 4 cas de goitre plongeant
endothoracique euthyroïdien opérés a montré des résultats encourageants.

Abstract :
The aim of this study was to report the results of surgery of endothoracic goiters in our department.
From February 2001 to April 2008 four women of respectively 36, 37, 43 and 48 years old have been
operated from an endothoracic and euthyroid goiter. Three patients presented anterior cervical
tumefaction; one had thyroid surgery five years before. All patients presented at least one mediastinal
mechanical compression symptom like dry cough, restrained dyspnea, dysphagy for solid food, thoracic
intermittent pain. The entire patient had a standard x-ray and cervico-thoracic scanner that confirm the
diagnostic. The surgical treatment consists in tumorectomy via cervico-sternotomy, once by median
sternotomy and two times by postero-lateral thoracotomy. The operatives immediates continuations
were simples in 3 cases, we note a persistent dysphony in one case and mortality was nil. The
anatomopathology’s exam of the tumour was in favour of a thyroid benign adenoma among all patients.
This short and homogen serie concerning 4 cases of endothoracic euthyroid operated goiter show
encouraging.
